Seeking a kind, reliable person with a social work or medical degree to meet weekly or bi-weekly with a 61-year-old woman with a history of homelessness and alcohol addiction in order to help her navigate towards health and wellness. She has a son living in the area who is actively involved in managing her care but is seeking support with this role. Case management activities during meetings with her include: making phone calls, scheduling appointments, exploring resources, providing support for her problem-solving and decision making, and (optional) transporting her to medical and counseling appointments. The part-time case manager will communicate regularly by email and phone with her son and submit a monthly invoice to him for up to 16 hours of in-person consultation, with up to 4 additional hours for research, advocacy work on her behalf, and communication. For any transportation to therapeutic appointments, mileage will be reimbursed at the state rate of $0.67 per mile. References required.
