Transitions is looking to fill a full-time (35 hours per week), Case Manager II position for our Miryam’s House location. Miryam’s House is a one-of-a-kind transitional housing program where you will work in supporting women with life skills, celebrations, daily living skills, community dinners, and grocery shopping; providing case management to our clustered site and to alumnae of the program; maintaining organization and cleanliness of the home; and billing Medicaid/FCS.
