Clinical Coordinator - CCSJob DetailsJob LocationWHCG - Milwaukee, WIPosition TypeFull TimeTravel PercentageMinimal; Less than 10%Job Shift1st ShiftDescriptionJob Purpose :This is a professional position that provides clinical and administrative oversight of Comprehensive Community Services Case Management Program (CCS). This position provides direct supervision to the CCS staff members.Reporting RelationshipsThis position reports to the CCS Program Manager. This position provides supervision and guidance to the CCS Case Managers.Essential Job Functions :

Provide clinical supervision of CCS and staff. This includes team meetings, individual and side by side supervision.

Ensure adequate staffing of program and personally provide case management or cover any uncovered shift until coverage can be found.

Maintain caseload of clients and assist with teaming as needed.

Supervise staff according to commonly accepted supervisory practices. This involves ensuring that staff fulfills job duties at a satisfactory level of performance in all aspects of staff function ranging from appropriate communication with clients and other staff, to timely and complete written documentation.

Monitor and ensure that CCS and program meets all relevant local, state and federal standards, including HFS 63 and Title 19 regulations.

Maintain system of medical records management.

Complete all annual or periodic program reports for county, state, federal or internal agency needs in a timely manner.

Assist the program manager with the coordination of intakes of new clients to the program. This will include tracking of the intake process and completing initial paperwork for each new client as assigned.

Assist the program manager with the auditing of case notes to ensure accuracy and timely completion.

Assist the program manager supervisor in the discharge of clients to include tracking of documentation and coordination with Milwaukee County.

Assist the program manager by tracking various tasks that include, but are not limited to, treatment plans and program enrollments.

Assess staff training needs and address such needs within budget constraints

Responsible for accurate documentation and maintaining case file as required by agency policy, government regulations and applicable local, state and federal codes.

Responsible for meeting department designated monthly billing quotas, EHR note entry within 72 hours and biannual treatment plans.

Attends and participates in staff meetings, in-service training seminars and conferences as required.

Responsible for adhering to agency policies and procedures while carrying out daily responsibilities.

Providing client coverage for other staff members as assigned.

Attendance at community based meetings as assigned including, but not limited to, MC3 Change Agent meetings.

Required Technical Skills, Experience, Education, and Credentials :

Master's Degree is required in the field of Social Work, Counseling, or related Human Services Field

State Licensure required (LPC or LCSW)

Minimum of 3,000 hours experience working with adults with severe and frequent mental illness.

Client transportation is a requirement for this position. A personal vehicle, a valid Wisconsin driver's license, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage.

Must be able to clearly convey complex instructions and ideas both in writing and verbally

Must be familiar with Microsoft Word and Excel.

Experience with individuals with dual-diagnoses is preferred.

Experience with motivational interviewing is preferred.

Certification as a functional screener is desired.

Motor vehicle record acceptable to program and/ or contractual requirements.

A valid Wisconsin driver's license, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage”.

Access to a personal vehicle.

Work Environment and Physical RequirementsThe work environment and physical demands described here are representative of those that an employee may experience or must do to successfully perform the essential functions of the job:

Ability to communicate effectively over the telephone, by computer and in person.

Ability to use a personal computer.

Ability to perform well in a fast paced work environment.

On occasion must be able to push/pull/lift/move objects weighting at least 20 pounds.

Must have the ability to climb at least two flights of stairs

Work is performed largely in the community but may at times include an office environment or other various settings.

Hours of work will generally be during regular business hours and average at least 40 hours a week.

This position requires participation in reoccurring ‘on-call’ duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends.

Must be able to safely push a person in a wheelchair on a level floor or on a wheelchair ramp in various weather conditions. Must be able to transfer a person from a wheelchair to the toilet, chair, or van seat/car with use of a gait belt and/or manual or power Hoyer lift

This position may work in a variety of indoor and outdoor work environments which may include, various outdoor weather conditions, loud noises, and fluctuating temperatures and the occasional use of stairs.
