Job Title: RN Program ManagerJob DescriptionThe Registered Nurse (RN) Program Manager is ideal for someone who enjoys leading while having the opportunity to provide hands-on care, engage in team collaboration, and be part of a mission-driven organization.Responsibilities

Manage Community Care’s Program of All-Inclusive Care for the Elderly (PACE) operations at the Sacred Heart Convent site, ensuring quality and compliance of care and service delivery.

Work collaboratively with leaders of the Sister’s community to ensure the quality of nursing care and service delivery provided to PACE members.

Serve as a consultant for the interdisciplinary team on member-specific issues as needed.

Conduct and prepare quality assurance audits/studies.

Work with the staff scheduler as needed to ensure continuity of member care and service.

Provide orientation, ongoing training, and feedback to direct and matrix reports to ensure their job responsibilities and performance expectations are defined and understood.

Responsible for administrative tasks, patient care, and program oversight.

Essential Skills

RN License

4 years of experience in human service delivery with at least 2 years of experience working with frail elderly individuals with dementia

2 years of leadership experience including direct staff supervision

Additional Skills & Qualifications

Experience in management, elderly care, human service delivery, program management, case management, and patient care
