Seeking P/T Support Specialist. Position has shared phone duties for incoming calls. Must be available to work minimum of 4 hours shifts between 8 and 5 M-F. or any flex schedule within M-F 8 -5
We are a care agency seeking an individual to assist members receiving services through the Division of Developmental Disabilities (DDD). You will manage the onboarding process and ensure compliance with required documentation , training certificates, background search, PCSP and authorizations from DDD is met. This is a position that requires constant communication and management of your team of members and families. mentors members weekly as needed with provider requirements including EVV training and compliance.
DUTIES:
Contact and Follow up with families to coordinate and complete intake procedures. Guide providers through the process.
Liaise with State to obtain and organize necessary documentation for providing care, including PCSP and authorizations
Required in home visitation for certain clients. 90 day visits for some clients is required.
Maintain accurate records and documentation of all communication during intake process
Data entry of vital information of all clients into state and proprietary systems.
Ensure compliance with all regulatory requirements and company policies.
Provide detailed orientation of all new hires.
Qualifications:
High school diploma or equivalent
Proficient with English - written and oral
Previous experience preferably in home health care or provider of HCBS
Familiarity with DDD services and related documentation processes is a plus.
Strong communication skills, both verbal and written.
Excellent organizational skills with attention to detail.
Ability to prioritize tasks and work independently within deadlines.
