Description The Care Manager will serve as a primary resource for clients and patients and clients within health and social services. This person will focus on a care plans for individuals , including those chronic health issues or the unhoused community.
Collaborate with clients, families, and interdisciplinary teams to set measurable goals for health improvement.
Conduct thorough assessments to identify each client's specific needs, resources, and challenges.
Act as a primary advocate for clients, ensuring their voices are heard and respected in all care decisions.
Use case management software to document all care activities and ensure data compliance with program standards.
Requirements
Minimum of 2 years of experience in case management, social services, or a related field. In-depth knowledge of local health and social service resources in Contra Costa County.
