CareMax Senior Solutions is looking for Care Coordinator in Fresno North area.
Utilize all systems used for hiring, scheduling, charting, and training
Reviews client profile and needs
Assesses caregivers and client matching
Manages scheduling of caregivers in a timely manner
Recruits, retains and train caregivers
Ensures that caregivers are up to date with licensing requirements and has all the necessary initial and annual training needed
Follow-up with client's care services
Follow-up with referral sources
Reviews all of caregivers shift reports
Provide immediate response when there is any changes in client's condition based on caregiver's reports
Be an effective liaison between referral sources, care staff and clients
Organize monthly staff meetings to ensure that staff/caregivers are consistently informed of company policy procedures, state regulations, and quality of care service provided.
Provide assistance with other administrative and care-giving roles as needed.
Requirements
High School Diploma or GED required. AA Degree minimum preferred.
Must have at least one-year experience in a healthcare setting required and preferably with scheduling experience
Must be able to work independently, manage multiple tasks while performing
work in a timely, accurate, and confidential manner
Must be computer savvy and have experience using Microsoft Word, Outlook and Excel required.
Must have effective written and verbal communication skills
Must be willing to go through background check and TB tests
Must have a valid CA driver's license and a reliable transportation that is insured
in accordance with state requirements
