Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Associate Manager of Medical Coordination is responsible for managing staff to ensure the discharge and oversight of medical care for homeless clients are being properly completed and ensuring medical operations are being completed in an efficient manner. The salary for this position is $75,000 per annum. This position will work the day shift.Responsibilities Managerial Duties:

Oversee medical operations for the facilities.

Maintain contractual and procedural compliance throughout the site.

Supervise and train Site Support personnel. Work closely with individuals to maintain and improve performance. Counsel and discipline staff members as needed.

Ensure schedules are optimized for productivity and approve timecards for accuracy.

Assist medical team with maintaining client work flows to ensure clients needs are met.

Attend meetings and relay pertinent information to medical team members.

Maintain accurate records in electronic and paper files.

Client Care:

Perform initial intakes to ensure proper client discharge from hospital with all appropriate needs for the continuation of care and documenting all findings.

Assess medical and mental health status of client upon arrival by conducting head to toe assessment, checking vitals, determining insurance status, making follow up appointments with clients’ doctors, and picking up/filling prescriptions as needed.

Verify staff complete morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.

Link clients to medical services when necessary (insurance, PCP, specialists, referrals).

Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health.

Exit clients by providing client with Exit Report that includes a list of current medications, future MD appointments, insurance information and other applicable medical information.

Collaborate with service providers to bring new resources for programs.

Report emergency situations to Supervisor and document incident

Seek proper level of care to assess and treat medical complication

Initiate medical response system when appropriate

Support client recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility.

Establish and maintain a care plan with client.

Administrative/Record Keeping

Enter client information and case notes into KIPU and other documentation systems.

Complete and update Medication, Vitals, and/or Diabetic logs.

Prepare client reports for billing.

Maintain proper inventory of medical supplies and medications.

Compliance/Safety

Maintain confidentiality in accordance with HIPAA guidelines and DHS standards.

Ensure clients are provided with proper compliance forms upon admission to maintain client compliance with program rules.

Provide care utilizing infection control measures that protect both the staff and the client according to OSHA standards.

Ensure completion of medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste.

Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and med storage room.

Ensure medical equipment remains in operating conditions.

Preferred Experience/Minimum Qualifications

Required:

Current and non-restricted LVN license

5 years relevant experience.

Demonstrated medical skills.

Customer and client management.

Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.

Valid CA Driver’s License and eligibility for company vehicle insurance.

Preferred:

2 years of managerial/leadership experience.

Bilingual in English and Spanish.

Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
