Chance 4 Change provides support services to adults with developmental disabilities.
Case Managers coordinate care including: medical, behavioral, financial, legal, residential and day-to-day needs. Case managers train and supervise support staff. In addition case managers collect and submit necessary documentation including data tracking and reports.
Management / supervisory experience along with prior experience working with disabled adults, physical and mental, is helpful. Criteria include: writing, typing and organizational skills along with computer literacy. Applicants need reliable transportation, current insurance pass a live scan and a willingness to travel to all surrounding towns of Eureka. Applicants also must be flexible, patient and positive
