JOB ANNOUNCEMENT
JOB TITLE: TOOTH CARE COORDINATOR
PAY RATE: $16.00 - $17.00 D.O.E.
STATUS: Part-time, up to 25 hours per week
AVAILABLE: Immediately

MINIMUM QUALIFICATIONS
At least two (2) years experience working with individuals and/or families in a social service program serving any low-income populations, and/or oral health practices
Desired Qualifications
Two (2) years college coursework in Social Work or related field
Bi-lingual in Spanish; be able to read, write and speak Spanish
POSITION PURPOSE
Under the general direction of the Community Services Division Director or their designee, the TOOTH Care Coordinator is responsible for providing relationship based, oral health case management services to children and their families. The TOOTH Care Coordinator collaborates with agencies throughout Humboldt County in an effort to best support children and their families with tooth decay.
ESSENTIAL JOB FUNCTIONS
Specific Tasks:
Attend children and family recruit events/activities
Provide families with oral hygiene and Dental Transformation Initiative (DTI) information and program criteria
Initiate contact and screen clients meeting Dental Transformation Initiative (DTI) criteria, entering them into the program database
Provide on-going case management by meeting with families who are participating in the DTI program
Strategize with clients to create a plan with realistic goals and timelines related to strengthening health related essentials
Assess and document progress toward meeting set goals and timelines
Assist clients with attaining goals and connecting to needed oral health resources and services
Provide client transportation to help with oral health related services when needed
Develop and maintain relationships with various staff partnering in the DTI program
Provide families with community referrals needed to reach their identified goals
Maintain on-going communication with clients over a 6 month period
Participate in staff trainings, DTI meetings, and in-service opportunities as directed
Assist in reporting and database management tasks as assigned or needed
Other duties as assigned or necessary.
JOB REQUIREMENTS
Knowledge of and Experience With:
Local community resources and services, primarily oral health and wellness
Effective methods of client engagement
Motivational interviewing techniques and theories; and creative ways of engaging individuals
Oral health and hygiene best practices
Networked computers, Microsoft Office Word and Excel programs, and Outlook Email
Ability To:
Understand poverty related issues and service models that support clients with a wide variety of complications accessing services
Work effectively under pressure with an ability to manage client needs and priorities
Build rapport with clients; provide role modeling through teaching opportunities; positive reinforcement; behavioral interventions
Communicate clearly and efficiently, written and orally; and be competent in English grammar, punctuation and spelling
Demonstrate good organizational skills and manage multiple tasks in an efficient manner
Develop and maintain cooperative and effective relationships with co-workers, RCAA Agency staff, personnel of other agencies, and local service population
Demonstrate strong interpersonal skills and ability to relate to individuals who may not share basic beliefs, including value systems and behavioral norms.
Work with culturally diverse communities and families, with the ability to be culturally sensitive and appropriate
Maintain a professional, confidential work environment and adhere to State/Federal HIPPA and laws/practices to protect client confidentiality
Establish and maintain personal and programmatic boundaries while providing support services
Ensure and protect agency, employee, program and client confidentiality and safety; and follow all protocols and procedures defined by this Agency and/or State and Federal laws to achieve this protection
OTHER REQUIREMENTS
Must be a U.S. citizen or lawful permanent resident, and have the ability to provide proof of identity and employment eligibility in accordance with Federal law
Must have means and capacity to perform job related duties with personal vehicle, as will be required.
Possession of valid Californias Drivers License, current auto insurance and acceptable DMV record.
Submit to background screening or fingerprinting for criminal record clearance and verification of your information listed on your application materials with acceptable results
