Under the supervision of the Aging & Disability Resource Connection Program Manager, the ADRC Service Coordinator will provide expedited access to services and support for individuals at risk of institutionalization, generally for 90-days or less, until a long-term plan is in place. Provides information and referral services, meets with consumers to complete intakes, and provides options counseling. Implements short-term service coordination using a person-centered approach. The ADRC Service Coordinator also coordinates with other extended partners to implement a “No Wrong Door” system of service delivery using shared data systems.
Compensation: $19 - $22 per hour
