Redwoods Rural Health Center (RRHC) is looking for an Outreach Case Manager. The Outreach Case Manager will provide outreach and case management services to individuals/households which are homeless or at risk of homelessness; to screen individuals and provide linkages between clients and appropriate services; to engage those with mental health issues, conduct intakes and assessments, provide case management, including housing placement assistance through available resources; participate in homeless services and collaborative case management meetings; and maintain appropriate client case file data in HMIS, client’s Electronic Health Record, and applicable spreadsheets and grant-prescribed data systems.
RRHC is an EOE and offers a flexible schedule and competitive compensation. Health benefits, paid time off and retirement match available for full and part-time employees working over 24+ hours per week. Pay range is $22 - $30 per hour depending on qualifications and experience.
