Description Join our dedicated Nonprofit , committed to supporting individuals experiencing homelessness in our community. We are seeking a compassionate and organized Case Manager to provide crucial support and guidance to homeless individuals, empowering them to navigate resources and achieve stability.Requirements Responsibilities:
Client Assessment: Conduct comprehensive assessments of clients' needs, strengths, and barriers to housing stability.
Individualized Service Planning: Develop personalized service plans in collaboration with clients, setting achievable goals and identifying appropriate interventions.
Resource Referral: Connect clients with community resources, including housing assistance, healthcare, mental health services, substance abuse treatment, employment support, and other essential services.
Advocacy: Advocate on behalf of clients to ensure access to necessary resources and support systems.
Crisis Intervention: Provide immediate assistance and support during crisis situations, including housing emergencies and mental health crises.
Case Coordination: Coordinate with internal team members and external partners to ensure seamless delivery of services and holistic support for clients.
Documentation and Reporting: Maintain accurate and up-to-date client records, progress notes, and service documentation in compliance with organizational standards and funding requirements.
Education and Empowerment: Offer educational workshops and support groups to empower clients with the skills and knowledge needed for long-term self-sufficiency.
Program Evaluation: Participate in program evaluation activities to assess effectiveness and identify areas for improvement.
Collaboration: Collaborate with other agencies, government entities, and community organizations to enhance service coordination and maximize resources for homeless individuals.
