They provide direct care to clients, working in a team-based environment and following through on all aspects of their care to implement treatment plans. The Social Services Assistant also serves as an intermediary between various healthcare team members, facility departments, and clients/families, addressing concerns and facilitating a positive treatment experience while ensuring compliance with protocols and facility standards.Key Responsibilities Serve as a contact point, advocate, and informational resource for clients, staff, treatment team, families, payers, case managers, and community resources, ensuring effective communication and coordination of care. Collect and analyze data on client care problems, stakeholder satisfaction, and admissions trends, reporting information to appropriate departments for action and improvement. Coordinate admissions and discharge processes, including paperwork, signatures, and obtaining necessary financial and risk factor information, while maintaining compliance with state and federal regulations. Conduct pre-admission conferences, provide tours and education to prospective clients and families, and initiate stakeholder satisfaction surveys to gather feedback and improve services. Assist with incident reporting, monitor chart integrity, and may provide client support for off-site outings and appointments as assigned.Qualifications & RequirementsEducation
Bachelor’s degree from an accredited college or university in Social Work or closely related field, or relevant experience.
Experience
Minimum of (2) two years’ experience with client centered care. One (1) year of experience in Mental Health highly preferred.
Licenses/Certifications
Current CPR/BLS certification (required)
Possession of a valid driver’s license as the position may require driving company vehicles. (Preferred)
Bilingual (Preferred)
Registered intern, licensed or licensed eligible as Psychologist, MFT, LCSW, LPCC (Preferred).
Completion of the core competencies of the Psychosocial Rehabilitation Specialist (PSRS), Mental Health Rehabilitation Specialist, and/or Clinical Specialist highly preferred.
Training or experience with electronic medical records (e.g. Cerner) preferred.
