DescriptionAs the Patient Records Abstractor, you will prioritize work and perform thorough reviews of medical record deficiencies anddelinquencies. This role is critical in meeting the Medical Center's data collection and coding needs, as well as ensuring compliance with third-partyregulations. You will:
Prioritize and perform reviews of medical record deficiencies and delinquencies.
Identify and address and clear the Discharges Not Billed (DNB) / Stop Bill Report for both inpatient and outpatient services promptly.
Manage the delinquent queries needed from physicians, including monitoring all queries generated within Medical Coding and CDI.
Hold cases for a predetermined number of days and escalate them to Medical Coding Management Team for resolution as needed
Collaborate closely with Health Information Management (HIM), Case Management, Utilization Review, Revenue Integrity, and Patient Business Services to facilitate prompt resolution of documentation and reduce liability for both the patient and the Medical Center.
Salary Range: $30.03-$39.63/hourlyQualificationsWe are seeking a detail-oriented and self-motivatedindividual with:
A high school diploma or General Education Diploma (GED), required
Minimum of two years of related hospital clerical experience, with at least one year working specifically with patient records
Knowledgeable in applicable medical terminology
In-depth understanding of the organization and significance of patient records
Familiarity with relationship between patient records and services provided to hospital business offices, insurance companies, Medi-Cal, Medicare, and other governmental agencies.
Ability to analyze, abstract, and compile medical information from patient records
