Here, patients find a conscientious environment and hope for a longer, healthier life.Essential Duties:As a Clinical Program Coordinator, Transplant (CPC) you will be the clinical expert who serves as the navigator and liaison between the transplant programs, the patients and the referring physicians; the CPC supports the vision of the medical and surgical program directors in planning continuing medical and nursing education opportunities in the greater Los Angeles area.
Creates networking opportunities for faculty and referring physicians in the community
Creates physician speaker series schedule and conduct presentations, maintain relationships with collaborative partners and cultivate new ones
Follow-up with physicians following a transfer or a referral
Rounds on inpatients to ensure smooth transition process
Participates in the development of future satellite clinics; serves as main point of contact between target facilities and CS
Ensures all CS logistical needs are in place prior to new clinic start date
Has a presence at all Satellite Clinics and provide patient education at these locations; deliver nursing care and complete initial patient intake and document in EMR
Organizes and coordinates CME courses and community lectures
Ensures education materials are current
Main resource at the satellite clinics between referring MD, clinic staff and CS admin and clinical teams
Maintains data tracking of referrals
Provides cross-coverage for other CPCs during vacation or leave of absence
QualificationsQualifications: Education:
BSN required
MSN preferred
License/Certification/Registration:
Current, unrestricted California RN License required
Specialty or Transplant Certification required or willing to obtain within one year from start date
BLS from the American Heart Association required
Experience:
A minimum of 3 years of nursing experience in transplant or acute care required
Demonstrated knowledge of case management required
Experience in Data Management required
Experience in coordinating large-scale community-based project
