Visit dignityhealth.org/northridgehospital for more information.Community Patient Navigator - Community & School Health InitiativeNorthridge Hospital Medical CenterNorthridge Hospital Medical Center is seeking a passionate and dedicated Community Patient Navigator to join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role for outreach and engagement to provide direct referrals to services for people who were the most affected by the pandemic. The goal is to further reduce health disparities by addressing social determinants of health, offering early intervention, and promoting health education.Responsibilities

Proactively reach out to newly diagnosed patients and their families to provide information and support.

Conduct initial assessments to understand individual needs and develop personalized care plans.

Build rapport and establish trusting relationships with patients and their families.

Guide patients through the complex healthcare system, including scheduling appointments, coordinating care, and navigating insurance processes.

Advocate for patients' needs and rights, ensuring they receive timely and appropriate care.

Provide emotional support and resources to patients and their families.

Connect patients with relevant community resources, such as financial assistance programs, transportation services, and support groups.

Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care.

Maintain accurate records of patient interactions and outcomes.

Contribute to program evaluation and reporting activities.

Other duties as assigned.

Qualifications

BS degree or equivalent college education and work experience required

6 months of experience working with the public or with community groups.

Excellent verbal and written communication skills and information management skills required.

Excellent organizational skills required.

Bilingual Spanish preferred

CA Driver License

Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
