Alto Lucero Transitional Care is seeking Full-Time Social Services - Case Manager to join our team! The Case Manager will be responsible for providing social services and case management to our short-term rehabilitation unit. This position reports to the Director of Social Services.
Responsibilities include:
⢠Interviews new residents and family to assess and develop plans to meet psychosocial needs
⢠Updates assessments and care plans
⢠Works with residents, representatives, and staff to coordinate discharge planning and services
⢠Act as resident's advocate, communicates with family, friends and other agents in the community as appropriate to make referrals for services
⢠Assist resident in obtaining services to maintain their everyday physical, mental, and psychological needs in accordance with state and federal guidelines
Skills/Requirements:
⢠High school graduate or GED equivalent
⢠Previous training and experience in Social Services preferred
⢠Good verbal and written communication skills
⢠Ability to maintain good interpersonal relationships with physicians, discharge, planners, social workers, and other community contacts
