Qualifications::

High School Diploma required, Associates of Arts or Bachelor Degree preferred.

Minimum of two years of experience providing direct patient services, including care coordination or case management, and working with patients from diverse backgrounds, ideally in a health care setting.

Essential Duties/Responsibilities:

Under the direction of the Admin Director of Integrated Behavioral Health, and to report directly to Lead Care Coordinator:

Develop, implement and conduct an outreach and education campaign that reaches new patients who are in need of mental health, substance abuse and/or opioid treatment, to include elements that serve to de-stigmatize issues related to behavioral health; presentations will also include information regarding case management services, linkages to care, and the availability of assistance in the enrollment process for Medi-Cal and/or other health insurance plans as appropriate.

In collaboration with the Axis outreach, development department and other care coordinators, develop materials that are appropriate to the needs of the project, to include ensuring that materials are culturally, linguistically and age-appropriate.

Coordinate and provide a referral process to link current, patients and newly identified community members to health enrollment and behavioral health care services, to include:

Conducting warm-handoffs internally in order to link patients to needed services.

Coordinating BH, MAT, and Maternal Mental Health referrals and appointments to the Axis enrollment and behavioral health departments

Assisting patients in completing forms and applications specific to enrollment and treatment services, or ensuring intake forms are completed.

Scheduling services as applicable. May include medication assisted treatment (MAT) follow up appointments, drug and alcohol intakes or follow ups, etc.

Providing ongoing communication with patients regarding scheduled appointments, the status of referrals, follow-up appointments and other issues related to accessing care.

Run reports for current patient recruitment as directed to increase utilization of chronic pain group, MAT utilization, etc.

Assisting in the arrangement for transportation services as necessary for patients that have transportation concerns.

Providing follow-up with patient to ensure that services were received and maintaining lists of patients who were unable to follow through for sporadic check-ins as necessary.

Providing follow-up with providers to ensure that patient reports are received and, as appropriate, provide follow-up regarding additional recommendations for care.

Review MAT charts for completion when necessary

Providing assistance to patients who encounter barriers to care.

Providing individual sleep coaching psychoeducation if appropriately trained by UC Berkley Sleep Team.

Provide back-up care coordination for other team members as needed.

Complete all required reports and ensure that all reports are submitted according to protocol in a timely manner.

Maintain patient records in Axis’s EHR according to protocol.

Maintain a high level of knowledge regarding: Behavioral health issues, to include opioid addition and treatment; Local resources; Medi-Cal and other public health insurance programs that are applicable to the target population; Policies and procedures and operational strategies that are specific to the program

As directed, provide programmatic information for Axis staff at staff meetings and in-service activities.

Serve as an advocate for substance abuse services for low income and uninsured residents in the Eastern Alameda County, to include participation in community forums, local government activities, school-based events and other community-based activities as assigned.

Attend and participate in weekly integrated behavioral health staff meetings and other project-based meetings as assigned.

Perform other duties as assigned.

Required Competencies:

Strong organizational, administrative and problem-solving skills, and ability to be flexible with change, while maintaining a positive attitude.

Excellent communication skills, to include public speaking skills.

Excellent customer service skills and the ability to communicate effectively and in a supportive manner with persons of all backgrounds.

Bilingual English/Spanish (written/spoken) desirable.

Ability to work independently and to plan, coordinate, prioritize and organize multiple job functions and activities

Detail-oriented, to include ability to perform reporting tasks efficiently and accurately

Ability and desire to work in a team-oriented environment

Understand concepts of integrated care services and experience using electronic health records is a plus.

Ability to work some evening hours required.

Physical Demands and Working Conditions:

Work is normally performed in a typical clinic office work environment (and, in some cases, telecommuting sites). The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this job successfully. Physical: Occasionally required to carry/lift/push/pull/move up to 20lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing, reaching over the shoulder, reaching over the head, reaching outward, sitting, walking on various surfaces, standing, and bending. Occasional travel to other Axis health centers and other occasional travel will be required.

Equipment: Frequently required to use repetitive motion of hands and feet to operate a computer keyboard, telephone, copier, and other office equipment for extended periods.

Sensory: Frequently required to read documents, written reports, and signage. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with staff etc. Must be able to speak clearly, understand normal communication, and be understood. Must be able to see clearly and have the ability to judge distances and spatial relationships to see objects where and as they actually are.

Cognitive: Must be able to analyze the information being received, count accurately, concentrate and focus on the given task, summarize the information being received, accurately interpret written data, synthesize information from multiple sources, write summaries as needed, interpret written or verbal instructions, and recognize social or professional behavioral cues.

Environmental Conditions: Frequent exposure to varied office (medical clinic/office) environments. Rare exposure to dust and loud noises.
