Registered Nurse - RN- No nights or weekends
Job Description:
Grace Adult Health Center in Santa Clara has openings for RN with case management and geriatric experience. Chinese Mandarin or Farsi language is a plus.
RESPONSIBILITIES:
Mostly documentations and vitals. Conducts nursing assessments of participants. Monitors health problems and develops and implements nursing care plans. Documents all treatments as they are provided. Monitors vital signs for each participant, including blood pressure, on a regular basis. Assists participants in maintaining optimum health, coordinating and supervising health-screening programs, assisting with dispensing prescribed medications. Provides health counseling to participants and their families on an individual and group basis. Develops and provides health-related training to staff, volunteers and families. Refers participants and their families to appropriate community health care agencies or facilities, acts as liaison with such organizations. Responsible for the coordination of personal care activities of participants including activities of daily living training and continence training. Supervises the program aides in their personal care duties and oversees the staff participating in personal caregiving. Acts as a nursing consultant for the staff and acts as primary liaison with the participant's physician when appropriate. Completes signed and dated quarterly progress notes, and six-month reassessments on all participants. Maintains current, written case management records; prepares a variety of written reports and correspondence. Serves as a member of the multi-disciplinary team and participates in the initial assessments, care planning, and on-going re-evaluation of participant's needs. Notifies participant’s physician of any marked changes in the participant’s condition.
QUALIFICATIONS:
Registered in the state of California. Recent case management experience with the geriatric population is preferred. Must have oral and written skills in order to prepare and present complete and concise written materials. Effective time management skills.

Salary: $45.00 to $65.00 /hour
