Then join our clinical team at our Santa Rosa Home Care Agency now . we need a RN or LVN as a Clinical Case Manager. This is a fulltime position Monday through Friday 8:30-5:30. We are locally owned by a nurse and our Agency does not accept Medicare.
Our clients are located in Sonoma County only. This is not a remote position. You will supervise our caregivers in our clients' homes, open new cases and provide skilled services and Case Management. (no experience in Case Management willing to learn we will teach and mentor you) NEW GRADs welcome - we will mentor and teach you. Full benefit package offered which includes mileage, health, dental, 3 weeks PTO and Company credit card for expenses.
