Our company is seeking a Home Health and Hospice Patient Service Coordinator to establish and strengthen relationships with community healthcare providers. A successful candidate should be outgoing, friendly, performance-driven, and have excellent communication skills. Being bilingual in Chinese or Vietnamese is a plus.
Responsibilities:
- To provide information to patients about Home Health & Hospice services (training available)
- Expand the company network of partnering providers
- Attend bi-monthly team meetings
- Plans and implements community service and community events, career fairs, and health care provider in-services
-Collaborate and participate in planning promotional activities and events
-Able to design and formulate marketing and outreach strategies
