Role: Independent Living Provider (Service Area: Oakland, Hayward, Richmond, Concord, Walnut Creek, Pittsburg, Antioch area)

As an Independent Living Provider, you're not just staff - you're a hero to our consumers. Your role is vital in empowering them to live independently and confidently.

What You'll Do:

Build and maintain professional yet compassionate relationships with our consumers and their support teams.

Conduct one-on-one skills training in line with each consumer's Individual Service Plan (ISP), both at home and within the community.

Ensure the health, safety, and well-being of consumers, minimizing any risks.

Support consumers by accompanying them to various appointments and helping them access community resources.

Participate in important meetings like ISP/IPP and Interdisciplinary Team Meetings to stay aligned with the consumers' needs.

Continuously evaluate and adapt programming to enhance consumer independence.

Provide First Aid and CPR in emergencies, offering peace of mind and safety.

Assist consumers in engaging with community resources, from identification to enrollment.

Recognize and address crisis situations, guiding consumers towards appropriate solutions.

Document progress and communicate effectively with the Happy Hearts Management team and funding sources.

Your Skills and Qualifications:

- A High School Diploma or equivalent, coupled with at least 6 months of experience in serving adults with disabilities.

- Excellent verbal and written communication skills.

- A knack for organizing, prioritizing, and planning effectively.

- Independence in your work, but also a team player.

- Commitment to professional development through regular training and meetings.

- A professional demeanor that represents the values of Happy Hearts.

- Current TB clearance, CPR and first aid certifications.

- A valid California driver’s license, clean DMV record, and current automobile insurance.

- Ability to clear a criminal background check and fingerprint clearance.
