Job description:
This position is responsible for providing direct support, positive re-direction, and assistance to adults with intellectual disabilities in accordance with day program policies and procedures:
Provides supports by assisting in the development and implementation of all support plans, assisting in achieving day program participants personal goals and desires, providing interaction and choices for activities that support a meaningful day.
Duties include but are not limited to:
Assists with information gathering for Personal Outcome Measures, and other assessments
Assists with maintaining a clean and safe environment
Maintains open communication and cooperative work effort with co-workers, supervisors, and management, to assure quality and continuity of support to individuals served
Reports and/or documents routine and unusual events including completing daily notes, special incident reports, data, etc.
Ensures that day program participants exercise their rights and that rights are not restricted and ensures that day program participants are treated with dignity and respect
Community outings, supervision
Dispense medication
Performs other duties as assigned
Preferred qualifications:
High school graduate or equivalent.
2 years experience with people with developmental disabilities/caregiving
Must be 18 years of age, or older
Must possess a valid driver’s license
Must possess verbal, reading, and written communication in English
Must have the ability to work effectively with others; be of good moral character; have an interest in the welfare of the individuals; and have a willingness/ability to communicate with and relate to individuals served, families, group home staff, co-workers, representatives of community agencies, company management, etc.
Must provide a negative Covid-19 test upon hire
