Job Description:
Able to work in small office setting non facility base
Work Closely with RN to provide quality care for Individuals with Intellectual and Developmental Disabilities
Performs client assessments based on agency and government standards of practice to ensure effective and appropriate care plans are followed.
Ensure that records are maintained, appointment/follow-up are scheduled.
Submit documentation to agencies that we are partner with.
Ensure that protocols are followed and that providers are trained to perform their duties according to protocols.
Understanding and knowledge of developing care plans.
Ability to establish and maintain constructive working relationships with individuals and their families.
Coordinates services with outside agencies.
Understands and adheres to established agency polices and procedures.
Conducts patient care conferences on patients assigned to his/her care.
Observes confidentiality safeguards all patients-related information.
Attends staff meetings and patient care conference as scheduled.
Completes documents and paperwork in a timely manner per agency policy.
Demonstrates competent performance of technical skills according to established procedures.
Understands and adheres to established policies/procedures.
Demonstrates sound judgement, critical thinking and decision making.
Organizational skills and time-management is a must.
Minimum Qualifications:
Current driver's license, safe driving record
Reliable transportation
Minimum of 2 Years LPN experience
Maintains current CPR certification.
Current LPN licensure with the State Board of Nursing in the state of Colorado
Performs other duties as assigned.
Provide QMAP training (company is already an Approved Training Entity)
