Integrated Care Management is seeking a Patient Care Manager to lead and coordinate care services for clients in need of compassionate, quality home care. The ideal candidate will be highly organized, proactive, and capable of managing multiple tasks in a fast-paced environment. This role requires a minimum of 5 years of office experience and hands-on knowledge of operations coordination.

Key Responsibilities:
Coordinate and oversee individualized patient care
Develop and implement plans of care
Initiate and manage treatment plans
Recruit, supervise, and support caregiving staff
Monitor care delivery and ensure service excellence
Conduct client intake assessments
Evaluate admission needs and eligibility
Provide follow-up and problem resolution for clients and families
Administrative Duties:
Answer and direct phone calls and emails promptly and professionally
File, organize, and maintain company records and project documentation
Write and distribute correspondence, memos, letters, faxes, and forms
Maintain client lists and client records
Order and track office and field supply inventory
Other administrative duties as assigned
Operations Support:
Organize and schedule Care, dispatch field teams, and track care progress
Maintain job calendars, work orders, and client documentation
Communicate updates between clients, technicians, and management
Assist with quality control and compliance documentation
Identify and resolve scheduling or operational conflicts
Candidate Requirements:
Minimum 5 years of office experience (required)
Operations experience is a must
Extremely detail-oriented, professional, and accurate
Excellent verbal and written communication skills to interact with clients
Excellent computer skills; proficiency in MS Office (Word, Excel) and Outlook required
Reliable, punctual, and able to meet deadlines
Strong time management and multitasking abilities
Team player with the ability to work independently
Strong conflict resolution and problem-solving skills
Eagerness to follow instructions and support management
Completion of a background check is required
Schedule:
Monday – Friday, 9:00 AM – 6 PM
Compensation & Benefits:
Salary: $50,000 – $70,000 per year (based on experience)

Qualifications:
Minimum Bachelor’s degree (4-year college graduate) in Nursing, Healthcare Management, Social Work, or a related field
Prior experience in care coordination, case management, or home health preferred
Strong organizational and leadership skills
Excellent communication and interpersonal abilities
Proficient in documentation and healthcare planning
Able to problem-solve and manage multiple priorities
