Submitted bid will show at minimum breakdown as follows:

Cost per hour – non emergency

Cost per hour – emergency

Printed name and Signature of authorized person submitting bid

Quarterly inspections and testing of fire alarm systems, emergency lighting and emergency EXIT sign lighting for (8) buildings

Annual inspections of all (3) backflow preventers with tags placed on systems, results sent to the city and given to the Maintenance Manager upon completion of test

Annual inspection of all fire extinguishers on Center with yearly tags placed on extinguishers (Approximately 100) also recharge if empty ones.

Annual Fire Hydrant Inspection (4 Fire Hydrants)

Semi-annual inspections of (4) kitchen and day care hood exhaust and suppression systems with tags placed on systems and results given to Maintenance Manager upon completion of test

Annual inspections of (1) fire sprinkler system and (1) FDC with tags placed on systems, results sent to the city and given to the Maintenance Manager upon completion of test

Vendor is to provide repairs to all systems and components 24 hours per day, 7 days a week. Vendor must have local stock of parts

Vendor must be prepared to correct all problems found during inspections at the request of the Maintenance Manager. Quote is required before work is corrected. The quote will be reviewed by FAD and once approved purchase order will be sent to vendor. Once the vendor receives the purchase order then problem can be corrected.

Vendor must respond to emergency calls within 2 hours of request and same day for all non-emergency needs.

Systems must be maintained in accordance with FL state fire codes and NFPA standards and codes.

Inspections and testing reports shall be presented in person to the Job Corp’s Maintenance Manager at time of inspection. One report is required for each master box.

Inspections will be by appointment only with the Job Corp’s Maintenance Manager.

Vendor shall hold a Florida license and all other licenses pertaining to above-described work. Licenses may be subject to review upon request.

In the event of an emergency, the Vendor will consult with the onsite Maintenance Manager or F&A Director to agree on estimated time and materials.

Fire Alarm System Service

Fire Pull station service and parts replacement.

Smoke detector service and parts replacement.

Labor Cost per Hour for Service Calls as needed to maintain system throughout the year addition to annual services.

To provide a schedule of when all services will be done.

Please itemized each service and cost on the bid.
