DescriptionWe'reseeking either a dedicated Part Time Social Work Case Manager MSW / LMSW / LCSWto continue the growth of our Social Work Case Management team at Emory University Hospital Midtown. As part of our team you will be responsible for coordinating clinically complex patient care needs across the health care continuum.Must have inpatient hospital experience.Our Social Worker will assess patients for transition needs including facilitating high quality clinical and cost outcomes; negotiating, procuring and coordinating services and resources needed by the patients in a timely and effective manner.Evening Shift: 2 12-hour weekend days 7am-7pm Saturday and Sunday and 1 12-hour night 7p-7am on Wednesday nights.MINIMUM QUALIFICATIONS

Must have a Masters in Social Work from an accredited Institution.

1 year recent healthcare experience preferred, experience in Acute Care setting preferred.

Must have working knowledge of software/Eemr applications. ACM, CCM preferred.

Must meet all quality and productivity expectations and successfully complete yearly competencies.
