Summary: The Patient Placement Specialist will be responsible for daily patient flow, acting as the central facilitation point for patients requiring admission within the Parkview Health System (excludes L&D, PBH, Ortho Hospital and Rehab/CCC). Performs initial high level assessment to all incoming admission requests, making quick initiation of emergent transfers. Recognizes appropriate specialty or service line needed in relation to patient condition. Facilitates and participates in physician to physician contact and hand off to assure patient safety during hand off with plan of initial clinical care. Works collaboratively with Point of Entry Case Manager to ensure all patients have verified admission order and status review. Monitors and projects bed availability based on known, predicted, or expected transfers and discharges. Prioritizes patient placement based on acuity, bed availability, staffing, and services required. Works collaboratively with internal and external interdisciplinary staff as needed to achieve accurate patient placement and patient flow optimization to assure that access to healthcare throughout Parkview Health is not compromised. Coordinates all in- house transfers and patient movement by triaging and supporting ICU capacity, OR throughput and ED decompression with the overall end point of supporting efficient capacity management. Responsible for assessing need, requests, clinical care requirements in all areas of patient care and prioritize those items to ensure that patient safety is not compromised. Oversees the Bed Coordinator. Education: Must have a BSN from accredited program. Acceptable accrediting agencies are Commission on Collegiate Education in Nursing (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA). Experience: Must have two years of medical, surgical, or critical care experience. Experience with bed tracking, scheduling, and ED software preferred. Experience in pre-certification, utilization, or case management desirable. Previous coding experience desirable. Other Qualifications: Excellent communication skills (oral and written). Qualifications:
Valid Indiana Registered Nurse license required.
Additional certifications may be required for specified areas.
Must be able to lift 50 pounds, assist with 100 pounds. (Lifting requirements may be heavier for specified departments.) Must have good verbal and written communication skills.
