Performs care coordination and professional nursing functions to assess the severity of the patient’s health concerns using protocols and algorithms in a manner that providers patient dispositions to emergency care and clinic provider appointments within the scope of the license practical nurse.Required SkillsCustomer Services:

Clinical Services:
Manages nurse-directed patient telephone calls requesting health care services.
Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols, guidelines, and standing orders.
Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.
Accurately documents symptoms / complaints, nursing assessment, advice provided and patient / caller response.
Coordinate and schedule same-day appointments on the providers’ schedules.
Manage prescription refills per protocol.
Provide patient education: e.g. chronic disease, preventative and family.
Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.
Environmental Services:
Contributes to the overall cleanliness and appearance of personal work space and department.
Required ExperienceMinimum Education: Graduate of an accredited nursing schoolWork Experience: Previous experience in medical practice is preferredRequired License/ Certification/Registration: Current Kentucky or Multi-State License. Basic Life Support CertificationSkills:
