That is why we are looking for a Program Aide to support our clients at our Adult Day Health Program in Brockton! Responsibilities:

Assist adult day health care members in personal care activities such as hygiene, grooming, dressing and personal care as needed.

Assist members in the practice of safety and good nutrition when possible.

Participate in assessment and care planning meetings and execute the plan of care.

Assist Registered Nurse with obtaining and recording vital signs.

Adhere to the documentation and care procedures and standards of personal and professional conduct.

Observe the patient for changes in general physical and mental conditions. Reports any changes noted to the adult day LPN.

Assist and supervise activities of daily living.

Assist with all the activities programs including arts and crafts and other small group activities under the direction of the Director.

Accurately documents care provided daily and maintains records per regulatory standards.

Assist with trips and other community outings outside the program.

Recognizes the physical, emotional and developmental needs of participants, working in a manner that respects participants, their privacy and their property.

Participate in staff meetings, in-service training, and other relevant training.

Prepare, serve, and clean up after meals and snacks.

Knowledge, Skills, and Abilities:

Bilingual in English and Cape Verdean Creole or secondary language preferred.

Previous experience in hospital, nursing home or in-home setting.

Has the ability to read and follow written instructions and document care given.

Has empathy for the needs of the ill, injured, frail and the impaired.

Possesses and maintains good physical stamina and mental health.

Demonstrates tact, patience and good personal hygiene.

Must be able to lift and carry up to 50 pounds.

CNA license or HHA certification required.

One or more years' experience working with adults in a health-care or social-service setting.

CPR Certification or willing to obtain.
