Early Intervention Physical Therapist

SUMMARY: The Pernet Family Health Service in Worcester is seeking to

fill a full-time EARLY INTERVENTION PHYSICAL THERAPIST who, under the

direction of the Program Director, will provide home-based services such

as developmental screening, assessment, direct service, and guidance to

families of children birth to 3 years of age who are at risk for, or

experiencing developmental delays; uses skills in an area of

specialization: education, nursing, occupational, physical, or speech

therapy, psychology, or social work. Wage from $28/hour; day shift with

occasional evenings and weekends. Work area includes Worcester, Auburn,

Boylston, Holden, Leicester, Paxton, Shrewsbury, West Boylston. DUTIES:

Provide home-based services to families across the catchment area

Demonstrate knowledge and understanding of all areas of infant-toddler

development Function as a multi-disciplinary team member in the

screening and developmental assessment of children birth to 3 years of

age who are at risk for or experiencing developmental delays

Participate in screenings, assessments, child groups, parent groups,

center visits, and home visits as a member of a multidisciplinary team

Utilize a parent coaching model in working with families Provide

service coordination to assigned caseload including coordinating the

development, monitoring, and review of the Individual Family Service

Plan With parental permission, collaborate with other community

agencies to ensure continuity of services for families including

childcare providers, medical providers, social service agencies, and

specialty providers serving children with autism or hearing/vision loss

to support the child and the family; empowers families to advocate for

their rights and needs REQUIREMENTS: Current licensure as a Physical

Therapist by the Massachusetts Board of Registration of Allied Health

Professions 2 yrs. pediatric experience (preferred) Must have

effective verbal and written communication skills Must have working

knowledge of Google Drive, Word, Excel, and Office 365 including

Outlook, OneDrive, Zoom and Teams Must have ability to use

computer-based technology including a computer/laptop and cell phone

Must have valid Driver License and reliable transportation
