If you are passionate about making a difference in the lives of individuals and families, join our mission-driven team and experience a career well lived. $5,000k Sign-on Bonus Early Intervention Physical Therapist Responsible for coordinating treatment and implementing services for assigned clients (includingoutreach visits) as a member of a multi-disciplinary Early Childhood Services team.Primary Functions:Provide outreach services to children and familiesConsult with team members in the development of individualized plans and multidisciplinary services for assigned clientsMaintain client records completely and accurately to include IFSP’s with full assessment of client functioning and goals,Documentation of client progress in the program, records of required contact with collaterals and family members,Documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clientsComplete billing documentation according to schedule established by the companyAdhere to professional standards and guidelines according to their respective disciplinesKeep supervisor informed of progress of all clientsMeet direct service requirements based on part time or full time status.Adhere to clinical recommendations from clinical administrationMaintain confidentiality in all clinical mattersRemain compliant with billing and documentation in accordance with direct service requirementConduct visits with clients and relevant collaterals in natural environments according to the needs of the child and familyQualifications:
