BHCC is an integrated care model which aims to improve behavioral healthcare access for patients with depression, anxiety and other common mental health diagnoses. This program emphasizes mental health as an important component of overall health and wellness. While our collaborative care model is a joint effort between the Department of Social Work and the Department of Psychiatry, this position is located within the primary care setting and the reporting structure is through the Department of Social Work. You will be responsible for:

Psychosocial evaluation, symptom monitoring, records review, administration of screening tools, care management, and care coordination, as well as engaging patients to develop self-management plans.

Case consultation with the psychiatrist. This will require preparation, accurate and up-to-date patient registry and records maintenance, and the ability to make case presentations accurately and concisely.

Use of brief evidenced based therapeutic interventions, psychoeducation, safety planning, assisting family members in understanding and supporting the patient, and providing referrals to community resources.

Working directly with patients face-to-face, virtually, and by phone. A vast majority of patient contacts are complete by phone.

Ongoing collaboration with the patient’s medical home team while drawing on social work experience and ongoing assessment of patient needs.

Required Qualifications

A Master's degree in Social Work from a school accredited by the Council of Social Work Education

A current license to practice social work in the State of Michigan

Desired Qualifications

Knowledge of methods of behavioral medicine and evidenced-based treatments for medical, mental, and substance use conditions.

o This position will be partly focused on alcohol related liver disease, so related experience in alcohol treatment would be strongly preferred.o This position is in medical settings and focusing on substance use and mental health conditions. An ideal candidate would have knowledge of and experience in medical, mental health, and substance use.

Experience consulting with psychiatric providers.

Efficient, accurate assessment of patient needs and effective case dispositions.

Skilled in developing rapport and effectively communicating with a diverse patient population.

Effective organizational skills, flexibility, self-motivation, and the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.

Independent, sound decision-making is necessary for success in this position.

Effective collaboration with a large multi-disciplinary team.

Proficiency in keeping accurate and timely clinical records by computer.

Experience with brief, structured interventions (such as Motivational Interviewing and Behavioral Activation).

Familiarity with evidence-based interventions (e.g., CBT, PST, ACT, DBT) and psychopharmacology.

Work ScheduleThis is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00. However, there is the possibility for flexible scheduling and remote work.Work LocationsThis position will be split between Alcohol Related Liver Disease Clinic and BHCC float. The ALD location listed is below and will require the SW to be onsite at least one day a week.Taubman Center at University HospitalFloor 1 Reception G1500 E Medical Center Dr SPC 5334Ann Arbor MI 48109-5334The BHCC float role will be primarily remote work and will provide coverage for leaves of absences, as well as high referral clinic areas for BHCC. This is currently a remote position but may be subject to change to include an onsite presence in the future.Underfill StatementThis position may be underfilled at a lower classification depending on the qualifications of the selected candidate.
