DescriptionJob SummaryUnder the supervision of the Data to Care Coordinator, the Housing Coordinator is responsible for supporting relationships and referrals between HIV service providers and housing programs in the City of Detroit.Examples of DutiesEssential Duties and Responsibilities (may perform other duties as assigned)

Partners with housing providers to understand housing options available to low-income clients.

Liaises between housing programs in Detroit to maximize usage of available housing resources.

Acts as the primary liaison between the Data to Care Program and the Michigan Department of Health and Human Services (MDHHS) to improve coordination of housing opportunities for people living with HIV (PLWH).

Attend city, regional and state meetings to improve coordination of housing services for PLWH.

Coordinates with landlords to increase the supply of affordable housing.

Minimum QualificationsQualifications (required):

Bachelor Degree required with 5-7 years of experience working in housing and/or providing services to individuals and/or PLWH.

3-5 years of experience as a Community Health Worker or in Case Management.

Experience working with the LGBTQ community.

Equivalent combinations of education and experience may be substituted to meet the education and experience requirements of this position.Qualifications (preferred):

Experience working with the Homeless Management Information System (HMIS) and/or CAREWare is a plus.
