We are seeking an Office Coordinator for Home Care/Private Duty with experience in home care tasks such as intake and staff coordination. Key responsibilities include:
- Managing service requests by phone or in person and processing referrals promptly.
- Verifying Home Help eligibility for new clients before services begin.
- Organizing and coordinating health fairs at senior and assisted living facilities.
- Keeping a client log and conducting daily follow-ups with clients, caregivers, and case workers as directed by the administrator.
- Preparing medical needs forms and delivering them to state or county offices.
- Handling client follow-ups, processing medical needs forms, and performing additional tasks as assigned by the administrator.
- Developing and maintaining relationships with group homes, senior living facilities, and hospitals for referrals and census growth.
- Carrying out other related duties essential for business success.
