Job Summary:Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.What you will need:

Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire; ADN only, if nationally certified in Case Management.

Current RN license in the State of Michigan.

Active BLS Certification

Preferred: Certification in Case Management or other related professional certification.

Two to five years of medical/surgery or chronic disease management experience in an acute care setting

Preferred: Previous case management experience, experience as a participant in continuous quality improvement, completion of self-management support training.

What you will work:

Full Time

40 hours per week

Day Shift

5 days per week, float between all inpatient units and ED.

Occasional weekends (approximately 6 weekends a year depending on staffing. In holiday rotation (approximately one holiday yearly)
