In this position, you will be responsible for identifying and providing for each resident's social, emotional, and psychological needs, and the continuing development of the resident's full potential from admission to discharge. Job duties include: Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident. Review all new admission paperwork and ensure all required documentation received for nursing and billing. Orient residents to the environment and facilitate adjustment upon placement. Assist residents and families with adjustment to changes, including room changes. Assist families with the engraving and/or marking of resident's personal items. Review residents' rights with residents and staff and attend to various personal needs of the residents. Assist in the development of a volunteer program and assist in facilitating family group meetings. Maintain point of contact with county case workers to ensure reimbursement documentation and medical appointments are up to date for all elderly waiver residents. Assist residents with appointment and transportation scheduling. Manage in-house psychology referrals and scheduling with outside agencies. Document progress notes that relate to each resident's care plan when necessary and within policy timeframe. Preferred Qualifications: Associate degree in social work or related field or two years equivalent experience in social services position in lieu of degree is preferred. Must be able to relate professionally, positively, and cooperatively with residents, resident's families, and employees. Must possess advanced computer skills.
