The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Montana State Hospital. The Montana State Hospital (MSH) in Warm Springs provides inpatient psychiatric treatment for adults with serious mental illness. MSH is the only public psychiatric hospital in Montana that serves people admitted from communities across the state. Behavioral Health Care Planners provide assessment, treatment planning, and supportive individual follow up to persons who are seriously mentally ill and are hospitalized. Provides case coordination/management and development of treatment plans and reviews. Develops and implements discharge plans. Acts to ensure functional communication of needs between the patient, the hospital, the family/guardian, and community providers/resources. Required for the first day of work: Knowledge of principles of social work, human behavior, contemporary social, and economic conditions, needs, and problems relating to the patient family background, and principles of federal and state legislation pertaining to social welfare community resources. Knowledge of treatment for patients with serious mental illness, as well as chemical dependency, developmental disabilities, personality disorders, etc. Effectively utilize knowledge and skills and apply them to practical assessment and treatment modalities for patients. Communicate effectively, verbally and in writing Ability to Analyze, assemble, and organize information.
