Job Summary:Provide comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning.What You Will Do:
Engage consumers and their families
Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness
Offer on-site as well as outreach services (including transport of consumers)
Fulfill case management services
Ensure consumers have community support in place that fulfill a positive corroborative role
Communicate observations and information about the consumer in the consumer's service record and to team members
Engage in supportive counseling and crisis intervention
Participates in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
Participate in quality assurance activities and cross-functional action teams
What We Need to See:
Bachelor's degree in a related field
Two (2) years experience working within a health care or behavioral health setting; Experience working with adults with acute symptoms of mental illness and with chronic health care conditions.
A valid driver's license and must be insurable by the agency's automobile insurance
