As a full-service home care & hospice agency providing a broad range of services to our customers, we are currently seeking a qualified and compassionate Registered Nurse (RN) Case Manager to care for our patients. Evaluate and treat patients using the most current technology and practices

Develop initial plan of care based on communication with the physician and patient assessment

Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives

Manage and educate Home Health Aides and LPNs performance in implementing nursing services

Collaborate with an interdisciplinary team to provide highest level of care

RequirementsQualifications & Requirements:

Current and unencumbered license to practice as a Registered Nurse in the state of New Mexico

Minimum of one (1) year of nursing experience

Current CPR certification

Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years

Exceptional customer care

Excellent written and verbal communication skills

MS Office skills (Word, Excel, PowerPoint)
