Exempt Up to $71,250 Job Summary: The co-located, collaborative integrated care behavioral health provider is a member of the PCMH health care team who assists the primary care provider (PCP) in managing the overall health of their enrolled population. The BHP's goals are to help improve recognition, treatment, and management of psychosocial/behavioral problems and conditions in the enrolled primary care Population. The person will provide consultation services to all patients referred by the primary care team. The person in this position will deliver brief, consultation-based services in the primary care clinic to patients and PCP's using an integrated care model as well as other services as recommended by the team and the PCP owner of care. Core Competencies:
Must be bilingual in English/Spanish.
Excellent communications skills, both written and verbal.
Considerable knowledge of various models of brief clinical interventions, crisis intervention, and short-term models using individual, group, and familial approaches in the primary care setting.
Meets specified deadlines and manages time effectively.
Exhibit skills in comprehending, interpreting, and completing delegated tasks efficiently.
Self-starter; must have the ability to work independently and follow-up on all work assignments.

Job Requirements:
Master's Degree from an accredited college or university in Social Work or another behavioral health related field and possess one of the following New Mexico licenses: LMSW, LMHC, LCSW, LPCC, LMFT.
