Reporting to the Assistant Vice President, the Program Director has overall responsibility for a program providing a full range of care management and supportive services to adults with serious mental illness to enable them to live independently in the community by securing permanent, supported housing in scattered site apartments and providing housing-related case management, care coordination and other support services. The Program Director supervises at least 2 supervisors and an administrative assistant directly in a growing program. Includes responsibility for managing Quality Improvement activities.Essential Duties:
Planning, developing, monitoring day-to-day operations of a 200-unit and growing supported housing program for seriously and persistently mentally ill (SPMI) persons, many of whom also have substance abuse disorders.
Supervising a staff including two Clinical Supervisors for the housing units who in turn supervise care coordination and supportive services, and one Administrative Assistant.
Maintaining contracted levels and standards of service.
Building relationships with other local agencies, such as state psychiatric centers, community residences and hospitals.
Communicating with OMH, HRA and other relevant state, city, and borough-based agencies, programs and service providers.
Working with landlords to secure appropriate and affordable apartments for clients.
Conducting site visits and emergency home visits.
Screening for appropriateness of placement and case assignment.
Coordinating and supervising all housing care management activities (service plans, locating apartments and moving, emotional support, preventing eviction, education about independent living skills (including house chores and budgeting) and safety, advocacy with landlords and housing courts, facilitating the clients’ rehabilitation process including referrals for psychosocial and vocational services and various housing options).
Organizing and participating in client case conferences.
Staff development, including new staff orientation, supervising staff, performance evaluations and staff training.
QualificationsEducation and Experience Requirement(s)Masters Degree in Social Work (MSW); LMSW or LCSW required. LCSW – R preferred. Five years staff supervisory and administrative experience is required preferably in a supported housing program.
