We are seeking a Registered Nurse Case Manager to provide comprehensive care coordination and management for residents in our supportive housing environments. As an RN Case Manager, you will be responsible for overseeing healthcare outcomes, ensuring that residents' needs are met, and working closely with care teams and families.Key Responsibilities:
Develop and implement personalized care plans based on each resident's unique health needs.
Ensure vital signs, basic assessments, and medication administration are completed by AMAP staff and follow up to confirm accuracy.
Serve as a liaison between families, healthcare providers, and outside agencies to coordinate care and communicate health updates.
Schedule and track appointments, consults, and documentation to ensure seamless coordination of services.
Complete ongoing assessments and documentation, including quarterly assessments for ICFS and bi-annual assessments for IRA residents.
Write Plan of Nursing Services (PONS), as well as weekly, monthly, and quarterly notes for resident care documentation.
Collaborate with Physical Therapists (PT), Occupational Therapists (OT), and Speech-Language Pathologists (SLP) from community providers as necessary to support resident care plans.
Participate in interdisciplinary team meetings as needed to review and update care plans.
Qualifications:
Current RN licensure in New York State.
Experience working with the special needs populations preferred.
Strong communication and organizational skills.
Ability to work independently after training and orientation.
Proficiency with laptops for documentation purposes; experience with electronic medical records preferred.
