Residential Case Manager Brooklyn, NY (http://maps.google.com/maps?q=657+East+98th+Street+Brooklyn+NY+USA+11236) DescriptionUnder the supervision of the Assistant Program Director, the Residential Case Manager isresponsible for, carrying a small caseload, provide crisis intervention, referrals to other levels of services and maintain up-to-date case records.RESPONSIBILITIES AND DUTIES:1.The Residential Case Manager is responsible for the completion of requireddocumentation in case records for residents assigned to his/her caseload.

Provide crisis intervention and brief counseling services for residents enrolled in the Brownsville Community Residential Program.

3.Refer residents to other level of services, i.e., outpatient rehabilitation programs, public assistance, medical, mental health and housing resources.4.Advocates on behalf of residents and their families when needed.5.Responsible for maintaining up-to-date case records in compliance withCity and State Regulations. 6.Foster abstinence and stability through counseling and other supportive services.

Completes other assignments as requested by supervisor.

RequirementsQUALIFICATIONS:1.Minimum of two years employment in an alcoholism/ substance abuse, mental health setting.

Credentialed Alcoholism and Substance Abuse Counselor (CASAC) or

CASAC-T eligible.Bachelors’ Degree a plus.Salary Description$47,104.98
