The Health Resource Manager is responsible for providing case management services for SVMC sub acute patients.Requirements:Graduation from an accredited school of nursing and a Bachelor’s of Science of Nursing preferred;applicants holding a valid RN license and currently matriculated into a BSN program may be considered.Prior experience in utilization review, discharge planning and/or case management preferred.3-5years of recent clinical experience required. VT Nursing License required by date of employment. Case Management Certification required within 2 years of employmentThe case manager is responsible for coordinating a patient-centered plan of care to meet individual needs by:
Completing intake interviews to identify a holistic plan of care; facilitating multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to discharge
Developing a patient transition plan, providing patient/caregiver education and proactively advocating for patient needs and goals across the interdisciplinary team to ensure an approach that is sensitive to each specific patient and family’s needs
Assisting providers in determining patient’s level of care in accordance with InterQual criteria
Actively seeking out collaboration with providers to improve overall quality and efficiency of care
