The Lead Case Manager is responsible for providing case management services for Southwestern Vermont Medical Center patients and overseeing the HRM and Social Services team.Requirements:Graduation from an accredited school of nursing;Bachelor's of Science in Nursing or in active matriculation required.Five years of clinical nursing experience required.Prior experience in utilization review, discharge planning and/or case management required.Ability to maintain a valid Vermont State Registered Nursing license.Case Management Certification with 2 years of hire is required.
