Competent in assessing client physical and psychosocial status changes and reporting findings to the Registered Nurse. JOB DUTIES/KNOWLEDGE

Provides skilled nursing to clients including, but not limited to treatment, medication administration, and treatment as directed by Registered Nurse and the Physician’s Plan of care in accordance with Agency policies and procedures.

Promotes and maintains client health through teaching and appropriate rehabilitative measures under the direction of the Registered Nurse.

Helps to achieve and maintain continuity of client care by completes exchanging information with other health professionals regarding patient care issues.

Completes clinical progress notes and follows client care plan as developed by the Registered Nurse and according to Agency policies and procedures.

Documents and reprints all changes in physical and behavioral status, all accidents, incidents, events, and injuries.

Follows agency protocols, policies, and procedures regarding infection control and disposal of biomedical waste.

Attends case conferences, meets with quality improvement coordinator(s), and communicates regularly with the COC as requested.

Provides for client safety; and responds to any signs of neglect or abuse by alerting the COC or PSM immediately.

Regularly updates personnel records and responds to requests made by the HR department.

Keeps abreast of changes and trends affecting nursing care and licensed nurse function(s), and attends mandated in-service classes as required by DOH and Agency policy.

Performs other duties as assigned by a COC, PSC, or PSM.

Requirements:

Active NYS LPN license.

Observe nurses and patients to ensure that proper nursing care is provided. 10. Prepare rooms, sterile instruments, equipment, and supplies, and ensure that stock of supplies is maintained. 12. Administer local, inhalation, intravenous, and other medications as ordered. 13.
