Directing the patients through the appointment, provisions of nursing services during care, and phone triage. Collaborates in care planning discussions, based on RN, MD or other Providers -determine urgency of care needs, triage of emergent care situations to the appropriate provider, and scheduling of acute care appointments to maximize patient care opportunities and unit efficiency. Maintains the flow of the Provider schedule during patient care sessions minimizing patient wait time whenever possible. Occasional evening and weekend may be required.Auto req ID:15358BRJob Requirements:Fulfilled the requirements to be a Licensed Practical Nurse in NY State. Minimum one year of LPN experience. Outpatient primary care and/or pediatric (if applicable) experience preferred. New York State licensed practical nurse. Current American Heart Association CPR and AED certification.
