The residents that reside on the unit are a mixture of LTC, Rehab, Dialysis, dementia residents. We at times have residents that are there for long term antibiotic use through a PICC line and are unable to be discharged with it. This unit has a relaxation suite that was intended to be a calming place that caters to dementia residents. It is a very resident interactive unit.STATUS: Full TimeLOCATION: Unity Living Center - 2300DEPARTMENT: Long Term CareSCHEDULE: ATTRIBUTESNursing Degree required, BSN preferredMinimum three (3) years acute/continuing care clinical experienceMDS proficiency preferredStrong leadership and communication skillsRESPONSIBILITIESDaily Management. Ensure safe and appropriate staffing levels and skills during all hours of operation; maintain ongoing communication with staff regarding performance; develop and assess department goals and objectives; act as a resource to department, staff, patients and other department staff and the communityLeadership. Act as a liaison and facilitate the flow of information; assure completion of incident reports and patient complaints with appropriate follow-up; support and communicate standards of care, management philosophy and objectives; provide input regarding departmental budget for staffing suppliesPatient Care. Develop the resident care plan and ensure implementation through the interdisciplinary team; develop, implement and evaluate in conjunction with Senior Director of a department Quality Improvement PlanPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)RN - Registered Nurse - New York State Education Department (NYSED)PAY RANGE: $84,000.00 - $115,000.00The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
